Age-Appropriate Transition Assessments & Career Plans

Teachers and staff: Please choose the correct Age-Appropriate Transition Assessment and tool based on your student's age during the IEP. 

Ages 12-14 Intermediate Age-Appropriate Transition Assessment- To be completed for students with a disability ages 11* (turning 12 in the lifetime of the new IEP) to 14.

Ages 15-16 Age-Appropriate Transition Assessment- To be completed for students with a disability ages 14* (turning 15 in the lifetime of the new IEP) to 16.

Ages 17+ Age-Appropriate Assessment- To be completed for students with a disability ages 16* (turning 17 in the lifetime of the new IEP) and older.
