Teachers and staff: Please choose the correct NYSAA Age-Appropriate Transition Assessment and tool based on your student's age during the IEP. 

Ages 12-14 Intermediate Age-Appropriate Transition Assessment- To be completed for students in the NYSAA program ages 11* (turning 12 in the lifetime of the new IEP) to 14.

NYSAA Ages 15-16 Age-Appropriate Assessment- To be completed for students in the NYSAA program ages 14* (turning 15 in the lifetime of the new IEP) to 16.

NYSAA Ages 17+ Age-Appropriate Assessment-To be completed for students in the NYSAA program ages 16* (turning 17 in the lifetime of the new IEP) and up.
